Personal Information Collection Statement
Purposes of data collection
1. The main purposes for which personal data held by Tobacco and Alcohol Control Office (TACO) of the Department of Health are or are to be used include –
(i) Personal Data for Service Provision and related activities collected from clients with whom the TACO interacts in the delivery of services and other related activities, such smoking cessation service, promotion activities, etc. The provision of personal data is voluntary. If you do not provide sufficient information, TACO may not be able to provide service or arrange activities.
(ii) Personal Data relating to Enforcement of Legislations collected from individuals during various enforcement activities. The provision of personal data is obligatory under the related Ordinances.
(iii) Personal Data for Personnel Management collected from staff and candidates for appointment for the purposes of manpower development and planning, employees’ appointment, performance appraisal and discipline, and so on. The provision of personal data is voluntary. If you do not provide sufficient information, TACO may not be able to process the appointment matter.
(iv) Personal Data of Visitors and Outsiders who made Enquiries to TACO collected from individuals who provided personal data when visiting and making enquiries/complaints to TACO. The provision of personal data for processing the suggestions, enquiries or complaints is voluntary. They will be used insofar as they are required for the related purposes.
Classes of Transferees
2. The personal data you provided are mainly for the use within the Department of Health but they may also be disclosed to other Government bureaus/departments, agencies or authorities for the purpose mentioned in paragraph 1 above, if required. Other than these, such data will only be disclosed to other parties where you have given consent to such disclosure or where such disclosure is allowed under the Personal Data (Privacy) Ordinance.
Access to Personal Data
3. You have a right of access and correction with respect to personal data as provided for in sections 18 and 22 and Principle 6 of Schedule 1 of the Personal Data (Privacy) Ordinance. Your right of access includes the right to obtain a copy of your personal data provided by you during the occasions as mentioned in paragraph 1 above. A fee may be imposed for obtaining a copy of the data.

4. Enquiries concerning the personal data provided, including the making of access and corrections, should be addressed to Tobacco and Alcohol Control Office of the Department of Health.
